Mrs. Slade, 75, died June 3, 2003, in Highland Health Care Center, Wellsville, after a long illness.
Born Mary Cannon in Mount Jewett, Pa., she graduated from Andover Central High School in 1944 and from Alfred University in 1948.
She retired from the Cattaraugus County Blind Association in Olean after a 40-year career.
She was a past president of the Andover Central School Board and the Allegany County chapter of the Mental Health Association. She was a board member of the Western New York Heart Association.
Her husband of 51 years, Raymond P. "Sam," died March 22.
Survivors include two sons, Daniel of Blasdell and Timothy J. of Belmont; a daughter, Ann of Andover; and six grandchildren.
